Mental illness is a serious condition with powerful consequences for individuals, their families and the community.  Family & Children’s offers a variety of services to aid our consumers in their recovery efforts.

Supportive and Intensive Case Management provides home-based services which include support and linkage to other services such as income assistance, housing, food stamps, Medicaid and medical treatment to increase the individual’s ability to live independently.  Services are offered on an outreach basis.

Who is eligible for Supportive Case Management?

Any Niagara County resident from the age of 18 and up who has been diagnosed with a serious and persistent mental illness may quality for Supportive Case Management (SCM) services.  Supportive Case Managers meet with consumers at least 2 times/month.

Who is eligible for Intensive Case Management?

Niagara County residents over the age of 18 who have been diagnosed with a serious and persistent mental illness and who frequently use emergency services or who have been hospitalized, incarcerated or homeless due to their mental illness may quality for Intensive Case Management (ICM) services.  Intensive Case Managers meet with consumers at least 4 times/month.

There is no cost to the consumer.  Funding for Case Management services is provided through the New York State Office of Mental Health and the Niagara County Department of Mental Health.  Medicaid is billed, if appropriate.
