Mental illness is a serious condition with powerful consequences for individuals, their families and the community. Family & Children’s offers a variety of services to aid our consumers in their recovery efforts. Supportive and Intensive Case Management provides home-based services which include support and linkage to other services such as income assistance, housing, food stamps, Medicaid and medical treatment to increase the individual’s ability to live independently. Who is eligible for Intensive Case Management?
There is no cost to the consumer. Funding for Case Management services is provided through the New York State Office of Mental Health and the Niagara County Department of Mental Health.
